Contracting with The Center For Sport Policy and Research (CSPR)

Vendors who complete work for CSPR in the amount of $500.00 or more will have to have a contract with the University before they can be paid. Contracts are customized through the University depending on type of work, length of contract, and amount paid.

Invoicing Instructions:

All invoices should be on letterhead. If letterhead is not available then the invoice should be signed by the person to receive payment. The invoice should include name, address, phone and fax of vendor (person receiving payment), the date of service/goods, the type of service/goods, and the charge for service/goods. Invoice should also include payment due date.

If a vendor has not conducted business with MTSU under the current vendor name then we will need a completed Substitute W-9 form. The link to this form is http://frank.mtsu.edu?~boffice/forms/w9form.pdf.  It can also be found by going to www.mtsu.edu, click on A-Z Index tab, Click on Administration on left side of screen, scroll down to Division of Business and Finance and click on Online forms-Business and Finance, click on Business Office Forms, scroll down to Accounting Services Forms, click on Interactive next to Substitute W-9 form.  Complete form as instructed on this page.

Finally, MTSU requires a completion of a Minority/Ethnicity Form- http://frank.mtsu.edu/~contract/minorityethnicityform.doc for all contracted employment.
